Physician Leader opportunity in Albany, NY

Responsibilities

• Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care
• Collaborate with leadership to review care coordination initiatives
• Develop intervention plans, and share ideas about network and risk management issues
• Work alongside pharmacists on medication management
• Implement an individualized patient care plans
• Timely response in chart completions and pharmacy recommendations
• Provide oversight and mentorship to our clinicians in the greater Albany, New York, area

Experience as a medical director in a skilled nursing facility preferred. However, clinicians who are board certified and experienced in the following disciplines are encouraged to apply: family (FM), primary care, internal (IM), emergency, (EM), adult, post-acute, home health, hospice, palliative, urgent care, and hospital medicine (HM).
